If anyone in your family had Marketplace health insurance in 2019, they probably already have. Form 1095-A, Health Insurance Marketplace® statement,on mail. Here are five important things to know about this form.

Helpful Tips for Tax Form 1095-A:

  • You should already have Form 1095-A. Obtained from the Marketplace, not the IRS. If your form was not mailed to you or you cannot locate it, please check your Marketplace account online before contacting the Marketplace Call Center.
  • Keep it in a safe place along with other important tax information, such as your W-2 form. This way it’s easy to find when you’re sitting in your files.
  • Before you do anything with the form, make sure it’s correct. Review your basic coverage and household information, and secondly review your Low Cost Silver Plan (SLCSP) premium. Contact the Marketplace Call Center Report an error.
  • If you request a new updated form, please do not submit it until the correct version has been submitted. If you have already filed, you may need to file an amended return using the information from your new 1095-A.
  • Use information from 1095-A to “adjust” the premium tax credit. To do this, compare the amount of premium tax credit you have used upfront for the year to the premium tax credit you actually qualify for based on your final income for the year.
