make an appointment

When booking, it is important to have your insurance card or other documentation ready in case you are asked to provide insurance information.

Here are five things you should do when making a reservation over the phone.

  • Let them know if you are a new patient. Especially new patients may have to wait several weeks to get an appointment. If you call a medical institution because you are not feeling well, you may be able to see a doctor on the same day.
  • Please tell us the reason for your visit. You may be looking for a new primary care provider, or you may need to visit us for specific concerns such as flu, allergies, or depression.
  • Please tell us the name of your health insurance plan. If you have Medicaid or CHIP coverage, please let us know.
  • Find out if you need to bring anything, such as medical records or current medications.
  • It’s important to know the name of the provider you want to see. Requesting a specific provider may result in longer wait times for your appointment. So if you don’t feel well and need to come right away, we may recommend another provider with openings in our network.
