If you have 2017 Marketplace coverage, you should have already received your Form 1095-A in the mail. Before you file your taxes, make sure your Form 1095-A is accurate.Use the information on this form
Complete Form 8962, Premium Tax Credit.
First: Verify Household and Coverage Information and SLCSP.
To confirm your SLCSP is correct, look at Part III, 1095-A, Column B, titled “Monthly Second Lowest Cost Silver Plan (SLCSP) Premium”. SLCSP Premium is incorrect if:
- Part III, Column B is ‘0’ or blank for months when someone in your household was on a Marketplace plan.
- There have been household changes that we have not communicated to the Marketplace, such as childbirth, moving, marriage or divorce, or loss of dependents.
