Health Insurance Marketplace® Form 1095-A and Income Tax Returns

If you or someone in your household has health insurance through the Health Insurance Marketplace® In 2014, a new Form 1095-A — Health Insurance Marketplace® Statement will be issued this tax season. It will arrive in the mail by early February and will be used to file your 2014 federal income tax return. Keep it with other important tax information, such as W-2 forms and other tax records.

When you receive your Form 1095-A, please verify that the information matches your records. Check the start and end dates of coverage, the number of household members, etc. If you think something is wrong, Contact the Marketplace Call Center.

What is on Form 1095-A?

  • Information about persons in your household who have taken out health insurance through the health insurance marketplace® 2014.
  • Information about monthly premiums paid for health insurance.
  • Prepaid premium tax credits paid for health insurance in 2014. These are deductible amounts that reduce the amount you pay in your monthly premiums.
  • The cost of the “benchmark” premium on which the premium tax credit is based. You cannot get this form if you have health insurance through work or through programs such as Medicaid, Medicare, or the Children’s Health Insurance Program (CHIP). If a family member switched plans or reported a change in her life in 2014, she may receive multiple Forms 1095-A. You will still receive a Form 1095-A even if you only covered part of the Marketplace in 2014.
